PHV was created to bring disciplined ownership, compassionate operations, and transparent reporting to small and medium-size assisted-living facilities. The work begins with residents and families, then extends to the investors who help make stable, well-run care homes possible.

Our care mission is to provide safe, attentive, dignity-centered assisted living for people who need support with daily life. Each facility should feel residential, responsive, and human, with skilled caregivers, administrator oversight, nutritious meals, medication support, activities, transportation coordination, and a clear path for families to ask questions.

Secondary mission: investors

Scalable operations with accountable reporting.

Our investor mission is to identify, purchase, improve, and manage assisted-living facilities through a repeatable process that can scale across markets. Investors should see property-level performance, occupancy, P&L, operating expenses, distributions, maturity windows, and the care metrics that make the business durable.

Strategic acquisitions

We evaluate residential care assets by location, licensing path, ALW relevance, capacity, staffing model, census opportunity, capex need, and operating upside.

Operations management

Facility performance is shaped by occupancy, caregiver coverage, resident acuity, food and supply costs, licensing, insurance, maintenance, and administrator accountability.

Market analysis

PHV monitors aging-population demand, local referral pipelines, payer mix, Assisted Living Waiver participation, and small-home operating economics before committing capital.
